Position Summary

A skilled and motivated Physical Therapist (PT) is needed for a rapid-response crisis assignment in a long-term care and skilled nursing environment. The PT will play a critical role in evaluating, planning, and executing therapy interventions to improve patient strength, mobility, and overall functional ability.

Key Responsibilities
  • Conduct comprehensive patient assessments to determine functional status and therapy needs.
  • Develop and implement individualized treatment plans to promote mobility and rehabilitation.
  • Monitor patient progress and adjust plans as needed to ensure optimal outcomes.
  • Accurately document patient evaluations, treatments, and progress in accordance with professional and regulatory standards.
  • Collaborate closely with interdisciplinary team members to support coordinated patient care.
  • Educate patients and families on therapy goals, exercises, and expectations.
  • Maintain a safe and therapeutic environment.
Qualifications
  • Active and unrestricted state Physical Therapist license.
  • Minimum of 1 year of experience, preferably in a SNF or long-term care setting.
  • Strong clinical reasoning and communication skills.
  • Ability to function independently in a fast-paced, rapid-response environment.
  • Commitment to excellence in patient care and rehabilitation outcomes.

  • Guaranteed 36-hour workweek.
