Enhance Rehabilitation is currently seeking qualified Physical Therapist candidates to join our dynamic team. As an Enhance Rehabilitation Physical Therapist, you will be part of a growing team providing the highest quality care for patients in SNF settings. In this role, you will evaluate and treat patients following an individualized plan of care. Responsibilities: Provide excellent care for our patients Deliver excellent customer service to patients and family members Collaborate with facility staff to ensure optimal patient care Evaluate new patients, develop plans of care, and set measurable, functional goals Treat patients, address their concerns, and educate them on their injury, progress, and probable outcomes Supervise Physical Therapist Assistants Re-evaluate patient progress, revise plans of care, and reset goals Complete timely and accurate documentation including evaluations, re-evaluations, discharges, and daily progress notes #J-18808-Ljbffr
