Physical Therapist (Day Shift)

• Guaranteed Hours: (36 hours/week)
• Compensation: Based on experience (Negotiable)

Job Summary:

The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete therapeutic process for an assigned group of patients in coordination with the physician and patient care team. The PT conducts patient assessments and participates in developing and implementing individualized care plans focused on goal achievement, patient engagement, and positive patient outcomes. The PT delivers or directs the majority of physical therapy services to support patients throughout the continuum of care.

Key Responsibilities:

• Provide evaluations and treatment interventions within the scope of practice established by law and professional standards.
• Assess and evaluate therapeutic, rehabilitative, and functional status, contributing to the development of the total plan of care.
• Deliver purposeful and skilled therapeutic care at the patient's home, utilizing techniques and procedures to produce measurable improvement consistent with the physician's plan of care.
• Educate patients, families, and caregivers on exercises, therapies, therapeutic appliances, and/or prosthetic devices for use outside of therapy sessions.
• Prioritize caseload, determine daily staffing needs, and coordinate assignments and handoffs.

Required Qualifications:

• Current Physical Therapy License (PT).
• Bachelor's Degree in Physical Therapy or related field.
• Minimum 2 years of experience in an acute care setting.
• Basic Cardiac Life Support (BCLS) certification (must be obtained within 30 days of employment).
