Job Summary (List Format) Physical Therapist - Home Health

- Setting: Home health, serving a rural territory
- Daily Caseload: Average of 6 patients per day, including visits, evaluations, admissions, and discharges
- Patient Population: Primarily geriatric patients
- Team Structure: 2 Physical Therapists, 1 Physical Therapist in orientation, 1 Physical Therapist Assistant
- Licensure/Certification Requirements: Active PT license, current CPR certification, valid car insurance
- Technology Used: Electronic Medical Record (EMR) system HCHB
- Coverage Area: Branch coverage for Cleveland, Gaston, and Rutherford counties
- Orientation: 5 days (facility and unit orientation)
- Schedule: Monday-Friday, 8:00 AM 5:00 PM; no weekend rotation or on-call required
- Holiday Coverage: As needed
- Time Off Requests: Approval required by manager
- Dress Code: Light gray scrubs
