The Home Health Physical Therapist provides skilled evaluation and rehabilitative interventions for patients in their homes, helping them regain mobility, function, and safety in the community. The role requires a clinically independent PT who is comfortable with OASIS, Medicare regulations, and point-of-care documentation in a geographically diverse service area.
Key responsibilities-
Evaluate home health patients and develop individualized, evidence-based plans of care that address mobility, strength, balance, and functional goals.

Provide direct physical therapy treatment in the home per physician orders, including therapeutic exercise, gait training, balance training, and fall-prevention strategies.

Complete OASIS Start of Care, Recertification, Resumption of Care, and Discharge assessments accurately and on time, collaborating with Quality and coding teams as needed.

Educate patients and caregivers on treatment plans, equipment use, home safety, and strategies to promote independence and reduce risk of rehospitalization.

Coordinate care with physicians, nurses, and other disciplines to ensure comprehensive, patient-centered services and consistent communication.

Supervise Physical Therapist Assistants (PTAs), including completing required supervisory visits and reviewing documentation in line with regulatory and agency policies.

Maintain timely, accurate point-of-care documentation using Homecare Homebase (HCHB), following agency workflows and compliance requirements.

Manage productivity expectations, daily time logs, and mileage records while optimizing routing and visit scheduling.

Participate in interdisciplinary case conferences, QA/QAPI initiatives, and other quality and performance improvement activities.

Graduate of an accredited Physical Therapy program (BS or higher).

Active Montana Physical Therapist license (required at or before start).

Minimum 2 years of Physical Therapy experience preferred, ideally with adult or geriatric populations.

Prior Home Health PT experience strongly preferred, including familiarity with home-based safety assessments and community mobility.

Proficiency with OASIS documentation and working knowledge of Medicare home health regulations and coverage criteria.

Valid driver's license, reliable vehicle, and appropriate auto insurance to travel between patient homes.

Strong communication, organization, and time-management skills with the ability to work independently in the field.

Home Health rehabilitation practice, including evaluation and treatment in the home setting.

OASIS & Medicare compliance, including accurate assessment, coding collaboration, and documentation of medical necessity and skilled need.

Care coordination and interdisciplinary communication across nursing, therapy, and provider teams.

Patient and family education on safety, mobility, and self-management strategies.

Independent clinical decision-making and problem-solving in variable home environments.

Ethical and professional practice, including respect for patient autonomy, privacy, and cultural considerations.
