Overview
We are seeking a compassionate and skilled Home Care Physical Therapist to join our care team in Fort Dodge, Iowa. In this full-time role, you will provide high-quality rehabilitative and preventative physical therapy services to patients in their homes. This position offers an opportunity to make a meaningful impact on patients' quality of life through personalized care, education, and support.

Key Responsibilities

• Conduct home visits for patients requiring home health physical therapy services

• Perform initial and comprehensive patient assessments

• Evaluate and document each patient's functional status and rehabilitation needs

• Provide treatments to relieve pain and restore movement and function

• Develop, implement, and update individualized plans of care

• Initiate and perform diagnostic, preventive, and rehabilitative procedures

• Ensure safe and effective delivery of all therapy services

• Promote patient independence through education and exercise programs

• Maintain accurate and timely clinical documentation

• Communicate with physicians and interdisciplinary team members regarding patient progress

• Supervise home health aides and physical therapy assistants as needed

Schedule and Shift Details

• Monday through Friday, 8:00 a.m. to 4:30 p.m.

Qualifications

Required

• Bachelor's degree or higher in Physical Therapy

• Active Physical Therapist license in the state of Iowa

• Proficiency with Microsoft Office and EMR documentation

• Strong communication, time management, and organizational skills
