Job Summary:
The Physical Therapist provides physical therapy services to patients in their homes within Hood River, OR, and surrounding areas. This role involves patient evaluation, treatment planning, and implementation of therapeutic interventions to improve patients' physical function and overall well-being. The therapist is expected to manage a caseload, maintain productivity standards, and collaborate with other healthcare professionals.

Responsibilities:
- Conduct patient evaluations to assess physical condition, functional limitations, and rehabilitation needs.
- Develop and implement individualized treatment plans based on patient assessments and goals.
- Provide therapeutic interventions, including exercise programs, manual therapy, and functional training.
- Monitor patient progress and adjust treatment plans as needed.
- Educate patients and caregivers on proper techniques and home exercise programs.
- Document patient care services accurately and timely in the electronic medical record (EPIC).
- Communicate with physicians and other healthcare professionals regarding patient progress and treatment recommendations.

Required Skills & Certifications:
- Active Physical Therapist license in both OR and WA.
- Basic Life Support (BCLS) certification.
- Minimum of 1 year of skilled home health experience.
- Oasis experience.
- Valid driver's license, car insurance, and reliable transportation.

Special Considerations:
- Traveler is expected to have their own transportation.
- The system groups clients to minimize drive time, but occasional travel within a 60-mile radius may be required.
- The facility can call off up to one shift per pay period (every two weeks).
- Confirm RTO restrictions with the hiring manager.
- Locals within a 50-75 mile radius are considered and accepted at the same rate.
- Traveler is expected to have their own transportation.
- The system groups clients to minimize drive time, but occasional travel within a 60-mile radius may be required.
