DUAL PRIVATE PRACTICE WELLNESS CENTER: Physical Therapist – 1:1 / Medicare Part B + Ortho Sports Performance Opportunity / AI NOTES
Lake Mary, FL $48 -$60 per hour | Flexible Part-Time or Full Time
Transitions Longevity Solutions (TLS), located within Transitions Physical Therapy & Wellness Center in Lake Mary, is expanding. We provide 60-minute, one-on-one outpatient care to active Medicare Part B adults who value strength, independence, and longevity. This is not a high-volume, double-booked model.
We are seeking an experienced Physical Therapist to help lead and grow our active aging division.

- 1:1 visits, no double booking, Medicare all 60- minute visits
- 2–3 day/week schedule, flexible scheduling or Full-time if your experienced in higher-level orthopedics, sports performance, or with advance certifications that match the needs of our private side
- AI-assisted documentation platform to reduce charting burden
- Private practice autonomy within a wellness-based environment
Primary Clinical Focus
- Active adults (Medicare Part B)
- Orthopedic and post-operative care
- Strength preservation and functional longevity
- Return to golf, fitness, running and travel activities
This role is ideal for a PT who:
- Understands Medicare Part B requirements
- Values clinical reasoning over high-volume productivity
- Prefers quality of care over quantity of visits
Qualifications
- Active Florida PT license
- Outpatient orthopedic experience
- Experience with Medicare documentation
- Manual therapy skills, dry needling or advanced certifications considered for cash pay private side
Practice Environment
- Private, clinician-owned practices
- Embedded within wellness center
- Collaborative culture
- Wellness center also has personal training, laser therapy, corporate wellness, massage, BHRT, women's health
Compensation
- Hourly for part time or pay per visit model
- Full Time Hourly base + performance bonus structure
