A supportive, employee-owned home care organization in Northern New York is seeking a dedicated Physical Therapist to provide in-home care to adult patients. Responsibilities:

• Conduct home-based evaluations using diagnostic muscle, nerve, joint, and functional ability assessments.

• Develop and implement individualized plans of care based on patient needs and physician orders.

• Provide direct therapy services to improve mobility, strength, and independence.

• Educate patients and caregivers on therapy goals, home exercises, and safe use of mobility aids or assistive equipment.

• Maintain accurate and timely clinical documentation for each visit.

• Participate in team meetings, case conferences, and in-service training.

• Adhere to safety standards and ensure equipment is properly maintained.

Qualifications:

• Graduate of an accredited Physical Therapy program.

• Current and valid Physical Therapy license issued by the New York State Department of Education.

• Minimum of one year of clinical experience as a licensed physical therapist.

• Strong communication, assessment, and organizational skills.

• Ability to work independently and manage a flexible home visit schedule.
