We are seeking a licensed Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility setting. The ideal candidate will be responsible for evaluating and treating residents to improve mobility, strength, balance, and overall functional independence.

• Perform comprehensive patient evaluations and develop individualized treatment plans

• Provide therapeutic exercises, manual therapy, and mobility training

• Implement fall prevention and strengthening programs

• Educate patients and caregivers on home exercise programs and safety techniques

• Collaborate with interdisciplinary healthcare teams to coordinate patient care

• Maintain accurate and timely clinical documentation

• Active and current state Physical Therapist license

• Minimum of 1 year of experience preferred, ideally in a Skilled Nursing Facility setting

• Strong clinical assessment and treatment planning skills

• Excellent communication and teamwork abilities

• Day shift schedule

• 36 hours guaranteed per week

• No holiday coverage required
