Our mission is to deliver exceptional, personalized care to clients in the comfort of their own homes. As a Physical Therapist, you will assess, diagnose, and create individualized treatment plans aimed at enhancing our clients' mobility and overall well-being. You will work closely with patients to motivate them through their recovery journeys and guide them in achieving their personal physical goals. In addition to hands-on treatment, you'll educate patients and their families on best practices in physical health, ensuring they feel empowered and engaged in their recovery process. Responsibilities

  • Conduct comprehensive evaluations and develop tailored treatment plans for patients.
  • Provide direct physical therapy services, guiding patients through exercises and recovery techniques.
  • Monitor and document patient progress, adjusting treatment plans as necessary.
  • Educate patients and their families about conditions, treatment options, and preventative care.
  • Collaborate with a multidisciplinary team to ensure holistic patient care.

Requirements

  • Current and valid physical therapist license in the state of practice.
  • Doctoral degree in Physical Therapy (DPT) from an accredited institution.
  • Proficiency in evaluating and treating patients with diverse needs.
  • Exceptional communication and interpersonal skills.
  • Commitment to patient-centered care and continuous professional growth.
  • Ability to work independently and in a team-oriented environment.
  • Willingness to travel to meet patients in their homes.
