Job Summary: The Physical Therapist is responsible for evaluation and rehabilitation intervention in the home setting, including management and supervision of patient care and patient and family education. Essential Duties and Responsibilities: Evaluates and establishes plan of care to meet patients needs in the home environment. Coordinates care with other disciplines and seeks orders for appropriate additional services. Provides direct patient care based on the established care plan and physician orders. Applies knowledge of physical therapy evidence-based practice guidelines, Medicare regulations, and Conditions of Participation in evaluations, re-evaluations, and treatment techniques. Modifies treatment as appropriate, including frequency and duration of care. Masters scoring of the CMS required OASIS and works with Quality team to collaborate needed adjustments. Acquires and maintains a basic understanding of the insurance requirements in Home Health. Is willing to perform interventions allowed under the physical therapy practice act, which may include but is not limited to wound care. Participates in multidisciplinary conferences to establish appropriate patient relevant care plans. Communicates appropriately with physicians and other staff members. Practices point of care documentation in all direct patient care settings. Documents all skilled services provided to patients, adverse events, and relevant communications in the medical record. Takes part in Quality Assurance Performance Improvement initiatives to assist the Agency in meeting its goals. Manages and meets productivity standards in line with the Agency s standards. Maintains accurate time logs and mileage. Follows agency policies for start and end of day commuting and logging of mileage. Communicates with scheduler on each day worked. Maximizes continuity of care and efficient travel routes when preparing schedules. Oversee services provided by the Physical Therapist Assistant and perform supervisory visits within required time frame. Takes supervision of CNA when RN is no longer involved in the care plan. Participates in relevant care conferences. Appropriately plans admissions, OASIS time points and discharges throughout the patient s episode of care. Maintains requirements of physical therapy licensure, including continuing education standards. Practices with expected standards of ethical behavior as defined by the physical therapy licensure board and Partners in Home Care policy. Participates in Agency provided educational opportunities as scheduled. Performs other duties as assigned by supervisor. Minimum Qualifications: Qualifications include BS degree or better from an accredited Physical Therapy program with current license to practice in the State of Montana, valid driver s license, and appropriate auto insurance. Two years of previous PT experience preferred.
