Cooperidge Consulting Firm is seeking a Physical Therapist for a premier Rehabilitation and Nursing Facility in Pawling, NY. This Direct Hire opportunity joins a compassionate, resident-centered clinical team dedicated to restoring independence and enhancing the quality of life for seniors. Located in the scenic Hudson Valley, the facility offers a warm, supportive environment where therapy is personalized and outcomes are prioritized. You will work alongside a multidisciplinary team to manage comprehensive rehabilitation plans, from pain management to mobility training, ensuring every resident achieves their highest functional potential. Responsibilities Clinical Evaluation: Conduct comprehensive initial assessments and develop data-driven, personalized treatment plans for short-term rehab and long-term care residents Mobility & Strength Training: Implement targeted therapy techniques to improve residents' range of motion, balance, and physical strength Pain Management: Provide specialized interventions using manual therapy, therapeutic exercises, and the strategic application of assistive devices Family & Resident Education: Act as a primary educator for residents and their families regarding therapy milestones, recovery expectations, and home-care transitions Interdisciplinary Collaboration: Partner with Nursing, Occupational Therapy, and Speech-Language Pathology teams to ensure holistic, patient-centered care PTA Supervision: Provide clinical oversight and support to Physical Therapy Assistants (PTAs), ensuring treatment protocols are executed accurately Compliance & Documentation: Maintain meticulous electronic documentation in accordance with New York State regulations, safety protocols, and infection control standards Requirements Education & Licensing Graduate of an accredited Physical Therapy program (DPT/MPT) is REQUIRED Current and valid New York State PT License is REQUIRED Experience & Skills Prior experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting is preferred Strong clinical reasoning and the ability to adjust care plans based on real-time patient outcomes Exceptional communication skills and a collaborative spirit Empathy and a passion for working with the geriatric population Benefits Comprehensive health, vision, and dental insurance plans Life insurance coverage 401(k) retirement plan with company matching contributions Paid time off including vacation, sick leave, and holidays Opportunities for career growth and advancement #J-18808-Ljbffr
