Key Responsibilities:

• Develop individualized treatment plans for patients in collaboration with healthcare team members.
• Provide hands-on physical therapy interventions and exercises tailored to patient needs.
• Evaluate and Monitor patient progress, adjusting treatment plans as necessary to maximize recovery and improve mobility.
• Educate patients and families on therapeutic techniques to support recovery and enhance independence.
• Document all patient care activities and progress accurately.
• Collaborate with other care team members, contributing to an integrated and supportive care environment.

Qualifications:

• Education: Bachelor's or Doctorate degree in Physical Therapy from an accredited institution.
• Licensure: Valid California PT license.
• Experience: Previous experience in a skilled nursing facility preferred but not required.
• Skills: Excellent communication and interpersonal abilities; compassionate, patient-focused approach to care.
