Home Health Physical Therapist – Role As a Physical Therapist in a home health setting, you'll work one-on-one with patients to restore mobility, reduce pain, and prevent falls, empowering them to live safely and independently at home. Key Responsibilities Conduct in-home PT evaluations and follow-up visits. Design and implement individualized rehabilitation and treatment plans. Improve patients’ strength, balance, mobility, and functional independence. Educate patients and caregivers on safe movement and equipment use. Document visits using WellSky Kinnser EMR. Coordinate care with physicians, RNs, case managers, and the interdisciplinary team. What We’re Looking For California-licensed Physical Therapist (PT). CPR certification (AHA-approved). At least 1 year of clinical experience (Home Health experience preferred). Organized, compassionate, and reliable. Valid driver’s license, auto insurance, and reliable transportation.
