We are seeking a dedicated Home Health Physical Therapist (PT) to provide high-quality care to patients in Sonoma, CA, and surrounding areas. The therapist will be responsible for evaluating patients, developing treatment plans, and implementing therapy sessions in a home health setting. This role requires strong clinical skills, self-motivation, and the ability to work independently while maintaining a patient-centered approach.

Provide one-on-one physical therapy treatment for up to 6 patients per day (30 productivity points standard).
Assess, plan, and implement individualized treatment programs for patients in a home setting.
Monitor patient progress, document therapy sessions, and adjust treatment plans as needed.
Educate patients and caregivers on therapy techniques to improve mobility and independence.
Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care.
Maintain accurate documentation in EPIC electronic medical records (EMR).
Ensure compliance with all federal, state, and facility regulations.
Drive to patient homes within an assigned radius (occasional travel of 50+ miles may be required).
Work one weekend shift per contract as required.

Qualifications

Experience: Minimum 1 year of skilled physical therapy experience (home health experience preferred but not required).
Certifications: Basic Life Support (BCLS) required.
License: Must have an active PT license or be eligible for a pending license.
Transportation: Must have reliable transportation for home visits.
OASIS experience preferred, but not required (please indicate if the candidate does not have OASIS experience).
Local Candidates: Accepted (considered local within a 50-mile radius; same rate applies).

Floating: No floating between units required.
Block Scheduling: Not available.
Guaranteed Hours: Facility may call off one shift per pay period (every two weeks).
Parking: Free.
Mileage Reimbursement: IRS Standard Rate.
Dress Code: No standard scrub color required.
