A supportive, employee-owned home care organization in Northern New York is seeking a dedicated Physical Therapist to provide in-home care to adult patients in the Saranac Lake area. Responsibilities:

• Evaluate patients using diagnostic muscle, nerve, joint, and functional ability assessments.

• Develop individualized treatment plans and modify care as needed in coordination with physicians.

• Provide direct home-based therapy focused on improving mobility, function, and independence.

• Educate patients and caregivers on home exercise programs and use of assistive devices.

• Maintain accurate, up-to-date clinical documentation for all visits.

• Participate in interdisciplinary case conferences, staff meetings, and training opportunities.

• Ensure patient safety and proper use of equipment during care.

Qualifications:

• Graduate of an accredited Physical Therapy program.

• Current and valid Physical Therapy license issued by the New York State Department of Education.

• Minimum of one year of experience as a licensed physical therapist.

• Strong interpersonal, assessment, and problem-solving skills.

• Ability to work independently and manage a home visit schedule efficiently.
