Physical Therapist OpportunityEnhance Rehabilitation is currently seeking qualified Physical Therapists candidates to join our dynamic team. As an Enhance Rehabilitation Physical Therapist, you will be a part of a growing team providing the highest quality care for patients in SNF setting. In this role, you will evaluate and treat patients following an individualized plan of care. You will provide excellent care for our patients, provide excellent customer service to our patients and family members, collaborate with facility staff to ensure excellent care for your patients, evaluate new patients, develop plans of care and set measurable, functional goals, treat patients, answer their concerns and educate them on their injury, progress and probable outcomes, supervise Physical Therapist Assistants, re-evaluate patient progress, revise plans of care and reset goals, and complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
