This full-time Physical Therapist role supports outpatient rehabilitation services on a day schedule. The position focuses on evaluating, treating, and progressing patients across a range of physical conditions while supporting functional independence and measurable outcomes.

• Evaluate and treat patients with orthopedic and mobility-related conditions

• Develop and implement individualized treatment plans

• Document evaluations, progress notes, and discharge summaries

• Educate patients and caregivers on recovery and prevention strategies

• Collaborate with providers and interdisciplinary teams

• Monitor outcomes and adjust treatment plans as needed

• Degree from an accredited Physical Therapy program (Bachelor’s, Master’s, or DPT)

• Active state Physical Therapist license (or ability to obtain upon hire)

• BLS certification (American Heart Association) upon hire

• Ability to manage outpatient caseload independently
