Key Responsibilities
Conduct comprehensive patient assessments and develop individualized care plans
Provide physical therapy treatments in patients' homes
Educate patients and caregivers on exercises, mobility strategies, and equipment use
Monitor patient progress and adjust treatment plans as needed
Maintain accurate clinical documentation and comply with regulatory standards
Participate in case conferences and interdisciplinary collaboration
Promote safety practices and proper use of equipment
Attend staff meetings and professional development sessions

Qualifications
Required
Graduate of an accredited Physical Therapy program
Current and valid New York State PT License
Minimum of 1 year practical experience as a licensed Physical Therapist
Strong communication, organizational, and patient engagement skills

Preferred
Experience in home health or hybrid home visit settings
