Job Summary:
The Physical Therapist provides home health services to patients, focusing on geriatric cases. This role requires a minimum of one year of recent experience, proficiency with HCHB EMR, and the ability to manage an average caseload of 5-7 patients per day. The therapist must complete all charting by 9 AM the following morning and maintain timely documentation and communication.

Responsibilities:
- Provide physical therapy services to patients in their homes.
- Manage a caseload of approximately 5-7 patients per day.
- Complete all charting in HCHB EMR by 9 AM the following morning.
- Maintain timely and effective communication with the healthcare team.
- Travel within a 60-mile radius from the hub/office to see patients.
- Adhere to the dress code of scrubs (any color, no jeans).
- Provide physical therapy services to patients in their homes.

- Active Physical Therapist license.
- BLS certification.

Special Considerations:
- Must be willing to travel up to a 60-mile radius from the hub/office.
