Job Summary
A full-time Physical Therapist opportunity is available in Waverly, NY. This role involves providing skilled physical therapy services within a collaborative care environment. Responsibilities
Develop and implement individualized physical therapy treatment plans based on physician orders
Complete evaluations, progress notes, and discharge summaries
Work closely with nursing and rehabilitation staff to support patient mobility and functional progress
Participate in interdisciplinary care meetings
Maintain accurate treatment and statistical documentation
Request necessary equipment and supplies
Provide training or education to staff as needed
Offer leadership support in the absence of supervisory personnel

Qualifications
Graduation from an accredited Physical Therapy program
Active New York State Physical Therapy license
Interest or experience in adult, geriatric, or subacute populations
