We are looking for a compassionate Physical Therapist to join our growing team! The right candidate has a can-do attitude, excellent interpersonal skills, and a current state PT license. In this role, you will be providing medical support to our patients while ensuring great customer service. Benefits Responsibilities Provides physical therapy services to patients according to a written physician's plan of care, including assessment and evaluation of therapeutic, rehabilitative, and functional status, and participation in developing the total plan of care. Evaluates home environment. Assesses muscle strength, mobility, gait, ROM, and potential for rehab. Directs physical therapy treatment. Instructs patients and families/caregivers in the use and care of therapeutic appliances. Determines priority needs for physical therapy. Reports to the physician on patient’s reaction to treatment or changes in condition. Initiates physical therapy programs and instructs other personnel and/or family/caregivers in phases of therapy, participating in case conferences. May train patients in the use of prosthetic devices. Identifies patient and family/caregiver needs for other home health services and makes referrals as necessary. Prepares and submits clinical and progress summaries based on goal attainment. Participates in discharge planning for patients. Provides physical therapy consultation to home families/caregivers when indicated. Provides in-service education programs for nursing personnel as needed. Participates in peer consultation processes. Supervises physical therapy assistants according to policy and regulations. When therapy is the only skilled service, instructs, supervises, and evaluates home health aide care biweekly. Qualifications Possesses a degree from an accredited physical therapy program. Licensed to practice as a physical therapist within the state. At least two years of relevant experience, including community/home health. Good verbal and written communication skills, organized. Current CPR certification.
