In this vital role, you will provide therapeutic services to patients with a focus on improving mobility, managing pain, and promoting physical function in a SNF setting. You will assess patients’ needs, develop personalized treatment plans, and implement evidence-based interventions tailored to each individual's goals. Your expertise will play a crucial role in helping our clients regain their independence and enhance their quality of life.

Schedule: Full-time

Key Responsibilities

  • Conduct comprehensive evaluations to assess physical function and limitations.
  • Develop individualized treatment plans based on patient assessments.
  • Implement therapeutic exercises and modalities to facilitate recovery.
  • Monitor patient progress and modify treatment plans as necessary.
  • Educate patients and families on exercises, techniques, and equipment usage.
  • Maintain accurate documentation in accordance with healthcare regulations.

Requirements

  • Current and valid Physical Therapist license in the state of practice.
  • Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited institution.
  • Previous experience in a clinical setting preferred.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively in a team-oriented environment.
  • Basic life support (BLS) certification preferred.

Benefits

$55hr-$60hr
