What You ll Be Doing

In this role, you will serve as the clinical anchor of a dedicated care team, overseeing the full continuum of patient health and wellness. Responsibilities include:

• Building and maintaining long-term, trust-based relationships with patients
• Managing chronic and acute conditions using evidence-based practices
• Conducting well visits and occasional house calls
• Providing after-hours phone support for continuity of care
• Monitoring both physical development and psychological well-being
• Leading a care team that includes a Registered Nurse and Patient Care Administrator
• Creating personalized health reports to guide patients and team members toward shared goals
• Participating in regular team huddles to proactively identify and address care needs

Ideal Candidate Profile

We re looking for a physician who blends clinical rigor with intellectual curiosity and thrives in a collaborative, high-touch environment.

Qualifications

• 5+ years of full-time clinical experience, including 3+ years in a high-volume setting
• Background in large hospital systems or similarly demanding clinical environments
• Experience working with underserved populations is a plus
• Academic credentials from top-tier U.S. or European institutions across at least two of the following: undergraduate, medical school, residency

• Detail-oriented with a passion for delivering seamless patient experiences
• Eager to stay current with modern diagnostics and emerging care models
