Job Description:
We are seeking a dedicated and compassionate Primary Care Physician or Nurse Practitioner to join our healthcare team in Bakersfield, CA, with a
focus on providing comprehensive medical care to the geriatric population. The ideal candidate will be committed to delivering high-quality healthcare services and promoting the well-being of our elderly patients. While experience with the PACE (Program of All-Inclusive Care for the Elderly) program is preferred, we welcome candidates with a strong background in geriatrics and a willingness to learn.
Responsibilities:
• Conduct thorough medical examinations and assessments of geriatric patients to
establish accurate diagnoses.
• Develop and implement personalized care plans, considering the unique needs and
conditions of elderly individuals.
• Collaborate with interdisciplinary teams, including nurses, social workers, and
other healthcare professionals, to ensure holistic and coordinated patient care.
• Provide ongoing medical supervision, monitoring, and adjustments to treatment
plans as necessary.
• Educate patients and their families on preventive care, treatment options, and
lifestyle modifications to enhance overall well-being.
• Maintain accurate and up-to-date electronic medical records, documenting patient
history, progress, and treatment plans.
• Participate in regular case conferences and team meetings to discuss patient care
strategies and coordinate services effectively.
• Stay informed about advancements in geriatric medicine and contribute to the
continuous improvement of healthcare practices within the organization.
• Adhere to ethical and legal standards, ensuring compliance with relevant
healthcare regulations.
Qualifications:
• Medical degree from an accredited institution and completion of residency in
Internal Medicine or Family Medicine.
• Board certification in Internal Medicine or Family Medicine.
• Current, unrestricted California medical license and DEA registration.
• Proven experience working with geriatric patients, with a focus on providing patient-
centered care.
• Familiarity with the PACE program is preferred but not required; willingness to
undergo training if necessary.
• Excellent communication and interpersonal skills to engage effectively with elderly
patients and their families.
• Ability to collaborate in a team-oriented environment with a patient-centered
approach.
• Strong organizational skills and attention to detail.
• Commitment to continuous professional development and staying current with best
practices in geriatric medicine.

• Excellent quality of life. Each provider:
• Only carries a panel of 125 to 135 patients.
• They see about 4-6 patients a day
• Mon-Friday, no weekends, and no after-hour on-call
