When a group of families came together in 1957 to find services for their children with developmental disabilities, ACLD was born. Today, we are a leading not-for-profit agency employing 1200+ staff and providing over 70 program sites in Nassau and Suffolk. ACLD assists children, teens and adults in reaching the highest level of independence with the freedom to ASPIRE HIGHER in their hopes, dreams and accomplishments. We are currently seeking a few exceptional people who have a passion to make a meaningful impact by facilitating the lives of the amazing people we support.

As a Program Coordinator, initiate and coordinate the development of all program and community related activities.

Specifically, you will:

  • Develop and coordinate the ongoing creation of the program’s meaningful community connections and community participation activities
  • Oversee the implementation of program activities that are consistent with the desires and interest of the individuals we support.
  • Monitor systems which promote organizational efficiencies as well as compliance with ACLD policies, procedures, and governmental regulations.
  • Ensure appropriate staffing ratios and provision of activities in accordance with individual Day Habilitation Plans.
  • Oversee the Life Plan process including meeting schedule and tracking system - Update and review assigned Day Habilitation Plans
  • Oversee the Developmental Disabilities Profile (DDP) process.
  • Coordinate and/or provide staff training including but not limited to: LITMOS, Behavior Support Plans, Diet Orders, and EVERO documentation
  • Coordinate and participate in transportation services for people supported in the Center-based program
  • Serve as a liaison between the program and families, residences, family care providers and the service coordinators to ensure a coordinated plan of care.
  • Represent the program by participating in presentations for potential applicants, families, faculty, staff, educational forums and school districts.
  • Coordinate the collection of community participation data. Analyze data to further develop community participation opportunities and to ensure maximum participation in the community.
  • Participate in the development of new program initiatives as well as current and future agency-wide initiatives
  • Monitor expense budget lines.
  • Maintain collaborative relationships with other ACLD programs and departments to maximize service coordination and integration.
  • Assist in the implementation of SCIP maneuvers when necessary.
  • Other duties, as assigned

We require:

  • Bachelors Degree in the field of Human Services; Master’s Degree preferred.
  • Three years working in field of Human Services with two years Supervisory experience required.
  • Obtain and maintain “SCIP” (Strategies for Crisis Intervention and Prevention) certification and AMAP training.
  • Complete required trainings in CPR (Cardiopulmonary Resuscitation).
  • Knowledge of OPWDD regulations and systems required.
  • Clean and valid NYS Driver’s License.
