ActivCare is seeking a Direct Support Professional for Seniors with Intellectual and Developmental Disabilities who are affected by dementia. The program specialist is responsible for the overall care and services to our specific residential population. Program Specialist / House Manager Responsibilities Learn our new process and be able to take ownership of this new program. Works with Program Director and other key staff to ensure proper oversight of residents is being carried out. Provide hands on care and work with other Care Providers to ensure residents' care needs are met. Prepare quarterly resident summaries, and attend all IPP meetings for each resident. Attend Staff meetings. Provide supervision, instruction, and assistance for residents as required in the Individual Program Plan or as needed. Maintain and/or cause to be maintained, all records (IPP Program, financial, medical, etc.) in accordance with regulations daily. Conducts audits and walkthroughs in assigned residents’ units to ensure compliance with Title 17 and Title 22. Facilitate individual and group outings while working with key staff members. Assist with group activities and meals. Work with Nurse and other Care Providers to ensure residents’ medical needs are met & in compliance with Title 17 and Title 22 regulations. Manage purchases and/or order forms to appropriate persons for: food, household supplies, paper goods, and managing resident's funds, submit P&I cash requests. Maintain family/conservator contacts. Other duties as assigned. Working Conditions This position will work in ActivCare Laguna Hills. Works primarily in a setting involving standing, sitting, bending, stooping and/ or crawling. This position requires using PPE when performing certain duties. May encounter bodily fluids which could potentially contain blood borne pathogens.
