Job Summary

Global Adult Care Services LLC is seeking a motivated and detail-oriented Program Specialist to support adults with intellectual and developmental disabilities. This role combines hands-on leadership, person-centered planning, and regulatory compliance to ensure high-quality services that promote independence, safety, and community integration. The Program Specialist serves as the Plan Lead, coordinating Individual Support Plans (ISPs), supervising documentation, training staff, and collaborating with Supports Coordinators, families, and service providers in accordance with PA Chapter 6400.

Key Responsibilities

• Coordinate and complete individual assessments in compliance with Chapter 6400

• Lead ISP development, implementation, reviews, and required meetings

• Ensure assessments, ISPs, and service delivery are aligned and implemented as written

• Track and analyze progress toward individual outcomes; complete monthly progress reports

• Recommend ISP revisions based on data and changing needs

• Maintain current ISP binders, health records, insurance documents, and Therap records

• Communicate changes in individual needs to Supports Coordinators and plan team members

• Coordinate and conduct ISP training for new and existing staff

• Supervise and review Direct Care Staff documentation (ISP data, T-Logs, SCOMM, appointment tracking, etc.)

• Schedule and attend medical appointments; ensure health logs and physician orders are current

• Oversee medication processes including MAR setup, refills, staff training, and error reporting (EIM)

• Review and balance participant financial binders per policy

• Support community integration and meaningful activities

• Research and coordinate new programs, activities, and community resources

• Participate in quality management, risk management, and incident management processes

• De-escalate crisis situations using approved techniques

• Assist with staff coverage during call-offs as needed

• Attend weekly program meetings and collaborate with internal and external partners

Qualifications (PA Chapter 6400.44(c))

• Master’s degree and 1 year of direct experience supporting individuals with ID/A

• Bachelor’s degree and 2 years of direct experience

• Associate degree or 60 college credits and 4 years of direct experience

Additional Requirements

• Knowledge of PA Chapter 6400 regulations preferred

• Strong organization, documentation, and communication skills

• Ability to work independently and as part of a team
