1. PURPOSE:

• This scope of work is for advanced project leader services to provide overall coordination, status reporting, and stability of project-oriented work efforts to support the CMS 3.0 Health Information Management System. Contractor will provide these services to the Florida Department of Health (Department).

2. TERM:

• This scope of work will begin on 7/1/2026 or the date on with the purchase order is issued, whichever is later. It will end at midnight, Eastern Time on 6/30/2027. The State of Florida's performance and obligation to pay under this purchase order and any subsequent renewal is contingent upon annual appropriation by the Legislature and satisfactory performance of the Contractor.

4. CRIMINAL BACKGROUND SCREENING:

• The Department will conduct a criminal history record check, including fingerprinting, on the consultant assigned by the Contractor prior to the consultant commencing work for the Department. The Department retains sole discretion as to whether the results of the criminal history record check will result in the Contractor's employee being disqualified from performing services pursuant to this agreement.

5. CONTRACTOR QUALIFICATIONS AND EXPERIENCE:

• Contractor staff assigned to this agreement must possess the following minimum qualifications and experience:
• 5.1. Bachelor's degree in computer science, Information Systems, Business Administration, or other related field, or equivalent work experience
• 5.2. A minimum of Seven years of IT work experience.
• 5.3. A minimum of Five years of project management experience.
• 5.4. Must possess advanced knowledge and proficiency in the use of project management methodologies and tools.
• 5.5. A minimum of Five years of experience with resource management includes day to day team monitoring and workflow management.
• 5.6. A minimum of Five years of experience with change management that includes requirement gathering, user story refinement, and documentation.
• 5.7. A minimum of Five years of experience working directly with project stakeholders and clients to plan and execute SaaS functionality features and enhancements.
• 5.8. A minimum of Two years of experience with agile project management.
• 5.9. A minimum of Two years of experience triaging application irregularities that included turnaround time monitoring and improvement measures.
• 5.10. A minimum of Two years of experience developing training and system guides for application end users.
• 5.11. A minimum of Two years of experience documenting application processes.
• 5.12. Experience with Children's Medical Services 3.0 Health Information Management System
