Psychiatric Mental Health Nurse Practitioner
Ithaca, NY & Surrounding Areas | Full-Time / Part-Time | Inpatient Settings
Cordial Health is seeking a compassionate and dedicated PMHNP to provide high-quality mental health services to residents in nursing homes, assisted living, and long-term care facilities throughout the Ithaca, NY region. This role is ideal for a practitioner with a heart for geriatric care and a commitment to supporting underserved populations with dignity, discretion, and clinical excellence.
Key Responsibilities
- Conduct comprehensive psychiatric evaluations and mental status assessments
- Develop and implement individualized treatment plans, including psychotropic medication management
- Perform follow-up visits and monitor treatment efficacy and side effects
- Collaborate with facility staff, primary care providers, and family members to coordinate holistic care
- Maintain accurate and timely documentation in compliance with regulatory and HIPAA guidelines
- Participate in interdisciplinary care team meetings and contribute to care planning
- Identify and respond to mental health crises or urgent psychiatric needs in coordination with on-site staff
- Educate facility staff on behavioral health issues and medication regimens as needed
- Attend weekly clinical supervision and case consultation sessions with Cordial Health's clinical leadership
Required Qualifications
- Active NYS license as a PMHNP
- Board Certification ANCC
- DEA license in New York State
- CPR/BLS certification
- Proficiency in psychiatric diagnostic tools and psychopharmacology
- Strong interpersonal, communication, and clinical decision-making skills
- Familiarity with EHR systems and documentation protocols
