Position: Public Health Representative 2

Position Summary:
The Dementia Program Navigator is responsible for promoting dementia-friendly practices within the community, conducting memory screenings, educating on Alzheimer's and dementia (ADRD), and supporting early detection and access to services. This role involves fostering partnerships with healthcare providers, maintaining client records, and tracking program success to ensure quality service delivery.

Key Responsibilities:

  • Community Engagement: Build relationships with partners, educate on ADRD, and support caregivers with resources.
  • Information Processing: Monitor program impact, maintain client records, and compile funding opportunities.
  • Knowledge Development: Stay updated on ADRD research, attend relevant training, and use data to guide outreach.
  • Compliance & Reporting: Ensure HIPAA compliance, manage budgets, and communicate with supervisors and peers for seamless program operation.

Qualifications:

  • Bachelor's degree in public health, nursing, social work, or a related field (or relevant experience).
  • Experience in public health, healthcare, or community health settings.
  • Knowledge in dementia care, HIPAA, client management, and community outreach.

Skills:

  • Strong communication and presentation skills.
  • Proficiency in active listening, critical thinking, organizing, and problem-solving.
  • Familiarity with office equipment and tools (computers, printers, etc.).

Competencies: Decision quality, customer focus, adaptability, and written/verbal communication skills.

This position requires an empathetic, organized individual with a commitment to enhancing dementia care in the community.
