By joining Epicured’s Medicaid & PACE team, you will play a direct role in expanding access to nutrition counseling and medically supportive food services for underserved populations across New York State. Role Overview Epicured is seeking Registered Dietitians to support our growing Medicaid and PACE programs, including services delivered under New York’s Section 1115 Waiver. Reporting to the Chief Product Officer, this role focuses on delivering high-quality nutrition assessments, counseling, and care planning for Medicaid beneficiaries enrolled in Epicured’s nutrition counseling and meal support services. This position is ideal for clinicians who are mission-driven, patient-centered, and experienced (or eager to grow) in Medicaid or community health settings. Key Responsibilities Conduct nutrition assessments for Medicaid beneficiaries referred for 3.1 (nutrition counseling) and 3.2 (meal box) services. Manage and follow up on existing Medicaid referrals, ensuring timely engagement and documentation. Provide individualized nutrition counseling, education, and care planning tailored to patient needs, goals, and social determinants of health. Deliver ad-hoc nutrition planning and consultation based on patient-specific clinical and lifestyle needs. Document patient interactions, assessments, and outcomes accurately within Epicured’s systems. Collaborate with internal teams, screening navigators, and program partners to support patient engagement and continuity of care. Support program goals related to patient volume, engagement, and outcomes tracking. Qualifications Licensed Registered Dietitian (RD) credential. Credentialed and authorized to practice in New York State. Up to date on all continuing education requirements per New York State regulations. Strong clinical judgment and patient counseling skills. Ability to manage caseloads independently in a fast-paced, evolving program environment. Preferred Qualifications Prior experience working with Medicaid populations or Medicaid-funded programs. Familiarity with social determinants of health and community-based nutrition resources. Bilingual or multilingual capabilities (Spanish, French, Hindi, Mandarin/Cantonese, Hebrew, or other languages a plus). Experience working within Section 1115 Waiver or similar value-based care programs.
