Purpose and Scope Provides nutritional services for the facility’s in-center and home patients to maximize their nutritional status and improve clinical outcomes. Works as a member of the interdisciplinary health‑care team, supporting the FMCNA commitment to the Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI). Actively participates in process‑improvement activities to help patients achieve individualized clinical goals set by their physicians. Principal Duties and Responsibilities Assesses patients’ knowledge of diet and kidney disease and provides education tailored to their learning needs. Provides detailed education to patients/families/caregivers about nutritional status, adequacy measures, fluid status, and lab parameters related to kidney disease. Completes nutritional assessments and participates in interdisciplinary comprehensive assessments. Assists in developing written, individualized comprehensive care plans that specify needed services in accordance with federal and state licensure guidelines. Calculates diet prescriptions according to standard of practice. Completes progress notes on all chronic dialysis patients per FMCNA guidelines, maintaining accurate documentation in the medical record. Provides ongoing nutrition assessment and outcome‑oriented counseling to help patients achieve and sustain effective nutrition. Identifies malnourished patients and those at risk, collaborates with the interdisciplinary team on interventions, and reviews albumin levels and body weight at least monthly to implement care plans. Monitors adherence and response to therapy, adjusts treatment as needed, and documents barriers to goal attainment. Collaborates with the team on Urea Kinetic Modeling and mineral metabolism, anemia, iron status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control. Follows FMCNA algorithms and medical record policies. Reports on nutrition QAI results and participates in the QAI program. Coordinates pre‑ESRD education as needed. Works with the Case Manager to coordinate education plans and identify candidates for nutritional supplements per disease‑management agreements. Reviews nutrition‑related lab results, counsels patients/caregivers, recommends treatment changes, and communicates findings to physicians and other care providers. Participates in relevant staff, quality improvement, and care‑plan meetings. Consults with the FMS Corporate Dietitian regarding nutrition concerns. Maintains and develops knowledge and skills for competent practice. Maintains dietetic registration and continuing‑education hours per American Dietetic Association and state licensure requirements. Ensures work areas are safe and clean. Performs other nutrition‑management duties as assigned. Physical Demands and Working Conditions Work environment typical of a health‑care facility with controlled temperature, moderate noise, and exposure to infectious materials. Desk and computer work, interaction with patients, staff, and physicians. Travel between assigned facilities and community locations, including regional, business‑unit, and corporate meetings (subject to reasonable accommodations for disabilities). Supervision None Education and Required Credentials Registered Dietitian per Commission on Dietetic Registration. Board Certified Specialist in Renal Nutrition encouraged. Current state licensure if applicable. Experience and Skills Minimum one year of professional clinical nutrition experience as a Registered Dietitian. Previous renal experience preferred. Demonstrated leadership competencies: teaching, organization, communication, customer service, relationship building, result orientation, team building, and decision making.
