Admission RN Home Health (South Park, PA)

Full-time | Estimated Salary: $80,000 per year

Responsibilities:
Manage and implement individualized patient care plans
Collaborate with the care team to coordinate and deliver effective care
Provide thorough and compassionate patient support in the home setting
Maintain accurate and timely documentation
Manage daily schedule and patient visits efficiently
Utilize tablets, mobile devices, and EMR software for charting and communication

Schedule:
Monday through Friday, 8-hour shifts
One evening on-call per week (5 PM 10 PM)
One weekend per month (Saturday/Sunday)

Qualifications:
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 1 year of experience using EMR systems
Valid driver's license
Current BLS and/or ACLS certification from the American Heart Association
