Responsibilities:
- Provide skilled nursing care to patients.
- Perform patient assessments and develop treatment plans.
- Administer medications and treatments as prescribed.
- Collaborate with the interdisciplinary team to coordinate patient care.
- Perform Care Coordination and Discharge planning.

Required Skills & Certifications:
- Active Registered Nurse license in the state of California.
- Clinical Assessment, Medication Administration, and Treatment Planning skills.
- Minimum of 2 years of acute care Hospital Case Manager experience performing Care Coordination and Discharge planning.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelor's degree in Nursing or related field.
- Driver's license, car insurance, and automobile for field work.
- Active Registered Nurse license in the state of California.
