Job Summary:
The Registered Nurse Case Manager is responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. This role involves patient assessments, medication administration, treatment planning, and collaborating with an interdisciplinary team. Strong communication and collaboration skills are essential for effective interaction with patients, providers, and team members.

Responsibilities:
- Provide skilled nursing care to patients.
- Perform patient assessments and develop treatment plans.
- Administer medications and treatments as prescribed.
- Collaborate with the interdisciplinary team to coordinate patient care.
- Perform Care Coordination and Discharge planning.

Required Skills & Certifications:
- Active Registered Nurse license in the state of California.
- Clinical Assessment, Medication Administration, and Treatment Planning skills.
- Minimum of 2 years of acute care Hospital Case Manager experience performing Care Coordination and Discharge planning.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelor's degree in Nursing or related field.
- Driver's license, car insurance, and automobile for field work.
- Active Registered Nurse license in the state of California.
