Job Summary:
The Registered Nurse Case Manager is responsible for care coordination and discharge planning for patients in an acute care setting. This role requires a minimum of two years of acute care hospital experience and strong skills in case management. The RN Case Manager will work as part of a team to ensure patients receive appropriate and timely care.

Responsibilities:
- Perform care coordination and discharge planning for patients.
- Collaborate with the interdisciplinary team to develop and implement patient care plans.
- Assess patient needs and develop appropriate discharge plans.
- Communicate with patients, families, and healthcare providers regarding care plans.
- Document patient care and progress accurately and completely.
- Participate in weekend rotation and meet departmental needs.

- Minimum of 2 years of acute care Hospital Case Manager experience performing Care Coordination and Discharge planning.
