We are seeking for an experienced RN Case Manager for a 13-week travel assignment in an acute hospital setting. This role requires a minimum of 2 years of acute medical Care Manager experience with a strong focus on care coordination, discharge planning, and utilization review. Prior travel experience is a HARD STOP requirement. Candidates must be proficient in Epic and able to manage an average caseload of 1:25 while floating to the Laguna facility as needed.

• Coordinate patient care across the continuum in an acute care environment

• Perform comprehensive needs assessments and develop individualized plans of care

• Lead discharge planning, including DME coordination and post-acute placement

• Conduct utilization review activities including appeals and denials

• Ensure compliance with CMS, DRG methodology, and payer requirements

• Collaborate with interdisciplinary teams to ensure timely and safe discharges

• Support disease management, disability, and workers' compensation cases as applicable

• Maintain accurate, timely documentation in Epic EMR

• Adhere to regulatory standards (CMS, DOH, HIPAA, OSHA, Joint Commission)

• Float to the Laguna facility based on operational needs

• Active California RN license (required)

• Minimum 2 years acute medical RN Case Manager experience performing care coordination

• Prior travel experience REQUIRED HARD STOP (no first-time travelers)

• Epic EMR experience REQUIRED

• Ability to manage 1:25 case ratios

• Willingness to float to Laguna facility

• Weekend rotation availability

• RTO limited to no more than 7 days

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)

• At least 2 years of acute hospital RN Case Management experience

• Experience with utilization review, discharge planning, and care coordination

• Acute specialty exposure preferred (ED, ICU, Med-Surg, Surgical Services)

• Active California Registered Nurse (RN) license

• Pending license accepted only if active prior to start date

• Acute care case management

• Care coordination and discharge planning

• Utilization review, appeals, and denials

• Needs assessment and DME ordering

• Knowledge of CMS, DRG, Joint Commission, and HIPAA requirements

• Workers' compensation and disability case management

• Epic documentation

• Strong interdisciplinary communication and time-management skills
