We are seeking for an experienced RN Case Manager for a 13-week travel assignment in an acute care hospital setting. This role requires a minimum of 2 years of acute medical Care Manager experience focused on care coordination and discharge planning. Prior travel experience is a HARD STOP requirement. Candidates must be proficient in Epic and comfortable managing an average caseload of 1:25 while floating to the Laguna facility as needed.

• Coordinate patient care across the continuum in an acute hospital setting

• Perform comprehensive needs assessments and develop individualized plans of care

• Lead discharge planning, including DME orders and post-acute placement

• Collaborate with interdisciplinary teams to ensure safe, timely discharges

• Manage utilization review activities including appeals and denials

• Ensure compliance with CMS, DRG, and payer requirements

• Support disease management and disability/workers' compensation cases as applicable

• Maintain accurate, timely documentation in Epic

• Adhere to regulatory standards (CMS, DOH, HIPAA, OSHA, Joint Commission)

• Float to the Laguna facility based on operational needs

• Active RN license (CA required)

• Minimum 2 years acute medical RN Case Manager experience performing care coordination

• Prior travel experience REQUIRED HARD STOP (no first-time travelers)

• Epic EMR experience REQUIRED

• Ability to manage 1:25 case ratios

• Willingness to float to Laguna facility

• Weekend rotation availability

• RTO limited to no more than 7 days

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)

• At least 2 years of acute care RN Case Management experience

• Experience with discharge planning, utilization review, and care coordination

• Acute specialty exposure preferred (ED, ICU, Surgical Services, Med-Surg)

• Active California Registered Nurse (RN) license

• Pending license accepted only if active prior to start date

• Acute hospital case management

• Care coordination and discharge planning

• Needs assessment and DME ordering

• Appeals, denials, and utilization review

• Knowledge of CMS, DRG methodology, Joint Commission standards

• HIPAA, OSHA, and Department of Health compliance

• Workers' compensation and disability case management

• Epic documentation

• Strong interdisciplinary communication and time management
