The East Bay team is looking for an experienced Nurse Practitioner to join our provider services team. JOB SUMMARY The Nurse Practitioner will support our mission by delivering competent, cost‑effective, timely, high‑quality health care for patients navigating the complex medical system. Effectively deliver the clinical and operational responsibilities of a 24/7 mobile medical clinic. Maintain performance and productivity standards, operational efficiency, excellent customer service, and best practices. Treat acute and routine medical conditions in person, by telephone, or via telehealth across all settings (e.g., SNF, ALF, house‑call visits). Demonstrate critical thinking, expert care, and best‑practice clinical decision making. Provide comprehensive care from history taking, clinical assessment, diagnosis, prescribing medications, to patient education and evaluation. Maintain compliance with state Nurse Practitioner practice standards and medical record documentation. Safeguard patient confidentiality at all times. Participate in professional development and maintain professional affiliations and CME credits. Ensure a safe work environment using appropriate PPE. Collaborate with team members and community partners (ALF, SNF staff, family members, home health and hospice team). Represent the agency professionally, fostering a cooperative atmosphere and maintain a positive, respectful demeanor. Operate with professionalism, independence, responsibility, and reliability. Prioritize workload and use work time productively. Remain humble and open to constructive feedback. JOB REQUIREMENTS Master's degree from an accredited Nurse Practitioner program, appropriate state licensure, prescriptive and APN authorization, and meeting credentialing requirements. Valid driver’s license with reliable transportation and willingness to drive in various weather conditions. Excellent verbal and written communication skills for interaction with partners, patients, families, physicians, and community representatives.
