This position focuses on coordinating hospice admissions and supporting patients and families through the initial stages of care. The role involves patient assessment, care planning, documentation, and collaboration with interdisciplinary teams to ensure seamless transitions into hospice services.

Key responsibilities

• Coordinate and deliver patient care plans during the admission process

• Support patients and families throughout Lords Valley and nearby areas

• Complete documentation and charting accurately and within required timeframes

• Manage time and schedule independently to accommodate patient needs

• Utilize tablets, mobile devices, and EMR systems effectively

Schedule

• Full-time position with flexible scheduling options

Travel

• Local travel required, approximately 20% of the workday

Qualifications
Required

• Associate degree or higher in Nursing

• At least 2 years of hospice experience

• Minimum 1 year of experience with Electronic Medical Record (EMR) systems, including TruChart EHR or similar

• Active Registered Nurse (RN) license in Pennsylvania

• Current driver's license

• Current American Heart Association Basic Life Support (AHA BLS) and Advanced Cardiac Life Support (AHA ACLS) certifications

Preferred

• RN license number provided for credential verification
