Job Summary:
The Registered Nurse will be responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Daily tasks include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Strong communication and collaboration with patients, providers, and other members of our team is critical to this role.

- ACLS certification
- BCLS certification
- NRP certification
- Advanced AWHONN certification
- LA Fire Card
- Active Registered Nurse license in the state of California
- Clinical Assessment skills

- Minimum of two years of hospital, home health, or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Bachelor's degree in Nursing or related field
- Driver's license, car insurance, and automobile for field work
