Job Summary:
The Registered Nurse will provide skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Responsibilities include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team, requiring strong communication and collaboration with patients, providers, and other team members. A minimum of 2 years of experience and exceptional skill is required.

Required Skills & Certifications:
- ACLS, BCLS, NRP, Advanced AWHONN, LA Fire Card, CA state license
- Clinical Assessment, Medication Administration, and Treatment Planning skills
- Minimum of 2 years of hospital, home health, or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Bachelor's degree in Nursing or related field
- Driver's license, car insurance, and automobile for field work
- ACLS, BCLS, NRP, Advanced AWHONN, LA Fire Card, CA state license
