Job Summary:
The Registered Nurse will provide skilled nursing care to patients in their homes within the Salem/Keizer, OR, and surrounding areas. This role involves patient visits, adhering to productivity standards, and collaborating within a team environment. The position requires a minimum of one year of skilled home health experience and proficiency with OASIS documentation.

Responsibilities:
- Provide skilled nursing care to patients in their homes.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Maintain accurate and timely documentation in EPIC.
- Manage a caseload of patients, potentially visiting up to 6 patients per day.
- Meet standard productivity points.
- Provide skilled nursing care to patients in their homes.

Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Minimum of 1 year of skilled home health experience.
- BCLS certification.
- OASIS experience.
- Driver's license, car insurance, and reliable transportation.
- Primary series COVID vaccination in addition to one booster.

Special Considerations:
- Traveler is expected to have their own transportation.
- Facility may call off traveler UP TO 1 shift per pay period (every two weeks).
- May need to work some weekends.
- Occasional maximum radius of 50+ miles for patient visits.
- No on-call requirement.
- No floating between units.
- No block scheduling.
