Job Summary:
The Registered Nurse provides skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness within a home health setting. The role involves patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team. The RN is expected to have their own transportation and manage a patient load that requires travel within a specified radius.
Location:Mount Angel, Oregon, United States
Responsibilities:
- Provide skilled nursing care to patients in their homes.
- Perform patient assessments and develop treatment plans.
- Administer medications and treatments as prescribed.
- Collaborate with the interdisciplinary team to ensure optimal patient care.
- Document patient care accurately and timely using EPIC charting system.
- Manage a patient caseload, potentially visiting up to 6 patients per day.
- Maintain required productivity points.
- Communicate effectively with patients, families, and healthcare providers.

Required Skills & Certifications:
- Active Registered Nurse license.
- BCLS certification.
- Minimum of 1 year of skilled home health experience.
- OASIS experience.
- Driver's license, car insurance, and reliable transportation.
- Primary series COVID vaccination in addition to one booster.

Special Considerations:
- Traveler is expected to have their own transportation.
- May need to work some weekends.
- Facility may call off traveler UP TO 1 shift per pay period (every two weeks).
- Occasional maximum radius of 50+ miles for patient visits.

Scheduling:
- 8-hour day shifts.
- 5 days a week.
- The facility cannot accommodate 4x10s or 3x12s schedules.
