Your actual pay will be based on your skills and experience — talk with your recruiter to learn more. Base pay range $70,000.00/yr - $100,000.00/yr Responsibilities Reviews referrals and assesses participant’s eligibility for admission in program. Completes full nursing assessment and develops medical plan for each participant. Assists treatment team in development and implementation of overall treatment plan. Actively participates as a treatment team member and performs related tasks. Provides medical case management including coordination of services with medical director, medical providers and other related service providers in the program and community. Acts as liaison with physicians, pharmacies and families regarding medical issues. Performs weekly rounds at program and more frequently as needed. Monitors medication administration and trains staff on medication handling, distribution and related policies and procedures. Trains and oversees medication coordinators. Assists with ensuring compliance with contractual, legal, and regulatory requirements. Performs all duties inherent in a supervisory role. Performs other duties, as assigned. Qualifications Completed Diploma, Associate, or Baccalaureate Degree in Nursing. Maintains unrestricted license to practice as a Registered Nurse in the state(s) to which assigned. Maintains current BLS (Basic Life Support) or CPR certification, in accordance state regulations. A minimum of two-year’s experience as licensed nurse. Case management experience and experience in residential environment preferred. Communicates effectively and professionally in verbal and written interactions. Demonstrates strong organization skills and attention to detail. Has reliable transportation and is able to travel to/from local program and community locations throughout work-day and week.
