Job Description:
This is a position supervised by the Residential Manager. The overall purpose of this position is to assure the health, safety and well-being of individuals and to assure appointments are completed as scheduled per DDA regulations. This includes, but is not limited to, assuring the implementation and delivery of services and supports designated in clients' Individual plans, provision of the individualized support, supervision, instruction, and follow-up needed to ensure individuals achieve and maintain the highest possible level of independence in functional life skills and monitoring and documenting individuals performance.
REQUIRED EDUCATION & EXPERIENCE
This position requires at a minimum a high school diploma or GED certificate. Some experience working with developmentally disabled individuals is preferred but not required.
This position requires successful completion of all DDA, and Agency required trainings.
Must have a valid driver's license with no more than 2 points and provide an annual driving record to Human Resources.
REQUIRED SKILLS
The ability to work and interact positively with individuals, co-workers, families, and individual advocates.
The ability to complete accurate and timely documentation
The ability to follow directions independently
The ability to respond to crises in a timely and effective manner
RESPONSIBILITIES & DUTIES
A) SPECIFIC
Provides clients with support, direction, & supervision to acquire appropriate functional life skills and to integrate successfully in the community. This includes but is not limited to, assuring participation in residential and community-based activities, encouraging relationships, & encouraging self-advocacy
Ensuring healthy meals are prepared for the person served.
Assisting persons served with ADLs as needed.
Communicating consistently and professionally with guardians, family members, and other personnel.
Assist individuals in maintaining a clean, neat, and safe environment; this includes but is not limited to, assuring that the residence will pass safety and environmental inspections. This will include laundry, house cleaning, dishes, and other environmental duties
Demonstrates knowledge of assigned individuals' goals, Goal Plans, Supplemental Procedures, Behavior Support Plans, and Nursing Plans of Care and assures they are implemented as written; maintains data as required/ specified in clients' PCP
Assisting with transporting individuals on appointments as needed on scheduled shifts and submitting documentation to appropriate personnel immediately after the appointment.
Assists individuals with any transitions such as Emergency room visits, residential moves, etc.
Assures individuals participate in all required/scheduled meetings, appointments, and daily activities
Conducts Just Community Meetings and ensures Events Calander is completed weekly
Checking PRN medications monthly to ensure none are expired and reorder as needed along with submitting a completed report to the Residential Manager.
Checking the disaster kit monthly to ensure each item is present and not expired.
Assures the Residential Manager is provided with a current telephone number by which the Lead Staff can be reached expediently
Attends meetings as scheduled or as directed by the Residential Manager or his/her designee.
Immediately reports incidents according to Agency policy. Is available to participate in the investigation
Must remain on shift until properly relieved by the next staff member
Maintains current certification in all agency-required trainings
Performs other duties as directed by the Residential Manager or his/her designee
Responsible for any funds received for household items and activities, turning in receipts to the lead/ Manager.
