Our employees make all the difference in our success!RoleThe Admission RN provides the primary coordination of hospital-based hospice and palliative care.The Admission RN receives all hospice and palliative care referrals and arranges for admission to thehospice or palliative consultative service, interfaces with the patient/family, and with the attendingphysician.The Admission RN triages all incoming referrals and coordinates interdisciplinary teammembers required to best meet the immediate needs of the patient.In collaboration with the hospice and palliative care medical consultant (when applicable), the admission RN initiates the care plan and discharge planning and communicates with the interdisciplinaryteam regarding patient goals and treatment plan.The Admission RN makes nursing assessments,judgments and treatment decisions based on customer needs and wishes and in accordance with thelevel of professional skills.The Admission RN promotes programs to acute care facilities through collaborative relationships with the facilityby providing opportunities for professional development and community education.The Admissions Registered Nurse (RN) in accordance with public health nursing standards, practices, and policies, will provide and arrange for Hospice care for patients/families, in a collaborative relationship, with other members of the interdisciplinary team. The Admission Nurse makes initial nursing assessments, judgments and treatment decisions based on patient/family needs and wishes in accordance with professional skills.Supervisory ResponsibilitiesThe Admissions RN is responsible for supervising LPNs, CNAs, Unit Secretaries, and Volunteers.Experience RequirementsThe RN must have at least two years of public health/community health, med surg or related nursingexperience. Experience in home care, hospice, and/or oncology nursing preferred.Education RequirementsThe RN must have at least a Diploma or Associates degree in Nursing from an accredited nursingprogram. BSN preferred.Required Certificates and/or LicensesThe RN must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice; or licensure from compact state. Healthcare Provider BLS (CPR) is required to practice in State of Maryland and the District of Columbia.Overall SkillsThe ability to demonstrate superlative oral and written communication skills as well as ability toorganize and manage projects within defined timeframe and budget; work both independently and aspart of a team; demonstrate excellent interpersonal and collaborative; demonstrate organizational skills with ability to delegate and follow up; manage multiple tasks and stressful situations with a positive attitude; demonstrate a strong work ethic and attention to detail.Physical/Mental/Emotional SkillsThe ability to possess exceptional skills, including a faculty for conceptual thinking, peer motivation,problem solving, and who can demonstrate initiative, follow-up and follow-through; work in a fastpaced environment utilizing tact, patience, and diplomacy; work independently; resolve conflict and be comfortable with conflict resolution and management; facilitate the development of trustingrelationships and partnerships with physicians, payers, patients and co-workers; maintain a positivepublic image for Capital Caring by dealing with patients and employees; communicate respectfully,openly, honestly and directly; display comfortably with possible blood, bodily fluid, communicablediseases, and etc. exposure; push and pull heavy objects; display a full range of body motion including handling and lifting patients; serve as coach, mentor and a positive role model; drive long distances, on a tight schedule, sometimes in heavy traffic; provide physical care to patients;Technological SkillsThe ability to learn and efficiently use patient health information systems and demonstrate proficiency in Microsoft Office and internet browsers.Reasoning AbilityThe ability to apply common sense understanding to carry out instructions furnished in written or oral form and deal with problems involving several concrete variables in standardized situations.Physical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
