Rn Case Manager Apply
Position: RN Case Manager
Location: Gainesville, FL 32614
Pay: $2002 per week
Shift: Day Shift (7:00 AM - 7:00 PM)
Duration: 13 weeks
Responsibilities:
- Provide case management services to patients, ensuring optimal care coordination and discharge planning.
- Assess patient needs and develop comprehensive care plans to promote effective healthcare delivery.
- Collaborate with multidisciplinary teams to ensure seamless transitions of care.
- Utilize Meditech and Midas systems to document, track, and analyze patient outcomes.
- Advocate for patients and their families, addressing concerns and facilitating access to necessary resources.
Requirements:
- Active RN license.
- Minimum of 2 years of case management experience.
- Proficiency with Meditech and Midas systems.
- Basic Life Support (BLS) certification.
- Strong organizational and communication skills, with the ability to work in a fast-paced environment.
