Charge Registered Nurse Skilled Nursing Facility (SNF)

Job Description:
We are seeking an experienced Charge Registered Nurse with a strong background in long-term skilled nursing care. This role is responsible for ensuring continuity of care by providing direct patient care while supervising and coordinating the work of CNAs, LVNs, RNs, and support staff.

The Charge RN will communicate closely with physicians and the interdisciplinary healthcare team regarding changes in residents' clinical conditions, diagnostic results, and responses to treatment. This position requires the ability to respond quickly and accurately to changes in patient status and to participate in Quality Improvement (QI) initiatives.

Key Responsibilities:

• Provide direct nursing care to residents in a skilled nursing setting

• Supervise and support CNAs, LVNs, RNs, and ancillary staff

• Assess, plan, implement, and evaluate individualized resident care plans

• Communicate changes in condition to physicians and care team members

• Ensure compliance with applicable regulations and standards of care

• Participate in Quality Improvement activities

Qualifications & Requirements:

• Active California RN license

• Minimum 2 years of long-term skilled nursing facility experience (required)

• Working knowledge of Title 22, OBRA, and other applicable regulations

• Experience with Electronic Medical Records preferred

• Current BLS certification

• Must pass a nursing medical competency test

• Strong verbal and written English communication skills

• Excellent interpersonal skills with the ability to interact professionally with residents, families, staff, and regulatory representatives

• Basic computer proficiency required
